Required Documentation

This checklist will assist you in providing the correct documentation to be reviewed and approved so that you may access disability services provided by Thomas Edison State College.

Disclosure of Disability (Appendix II)

Complete the Disclosure of Disability form in its entirety and return to the Office of Student Special Services via mail, e-mail, or fax.

Verification of Disability (Appendix III)

All students requesting accommodations under the ADA must provide documentation of their disability/ies by way of a written letter from their medical doctor, learning disabilities specialist, psychologist or evaluative agency.  Please refer to the “Verification of Disability” page in this packet with instructions for providing required documentation.  Once you have the information, please return it to the Office of Student Special Services via mail, e-mail or fax.

Term Accommodation Request Form (Appendix IV)

Complete the Term Accommodation Request form in its entirety for every term in which you are requesting accommodations and return to the Office of Student Special Services via mail, e-mail or fax.

Note:Your Term Accommodation Request must be completed after you know your mentor’s name and e-mail address and sent no later than the end of the second week of your term, as your mentor(s) must be notified.

Permission to Speak on Your Behalf (Appendix V)

Should you wish to permit someone else to speak with the College on your behalf, you must complete the Permission to Release Student Information to Another Source form in its entirety and return it to the Office of Student Special Services via mail, e-mail or fax.
