Section 3: Degree Information
List only one choice in each column. You may pursue a baccalaureate degree and an associate degree simultaneously. Please note that certain degree programs and/or major areas of study have special requirements for admission. If you are undecided or unsure, you may want to consider the Bachelor of Arts (B.A.) with a concentration in Humanities or Social Sciences.
If you are enrolled for a degree from another college, you must fill in the information requested. You may not earn two degrees from two different colleges in the same discipline or major area of study.
Section 4: Documents
In order to fully evaluate your academic standing, we will need copies of all academic records on file with your education center/office as certified by your education officer. These documents may include college transcripts, including transcripts from the Community College of the Air Force or U.S. Coast Guard Institute, AARTS transcripts, SMART transcripts, an original DD 295, service records, test scores for CLEP, DANTES and other examinations, and documentation of licensure. Your documents, along with this application and the required fees, must be compiled and sent by your Education Officer. All documents described in this section should be sent directly to:

Colleges and Universities: List the complete information on all regionally accredited colleges and universities that you have attended. If you have already completed a degree, you must provide that information. If you are transferring foreign credit, the College will accept credit recommendations, with a required course-by-course evaluation completed by any of the following agencies:

All costs associated with the international credit evaluation are the responsibility of the student. The College reserves the right to make its own determination on the amount and type of credit to be awarded based on the evaluations provide by these agencies. An enrolled student may transfer a maximum of 90 credits from international institutions. Thomas Edison State College does not participate in the Student and Exchange Visitor Information System (SEVIS). Therefore the College does not sponsor foreign national students for F-1 or J-1 visa status. Course-by-course credit recommendations will be reviewed by Thomas Edison State College based upon existing transfer and degree policies in place at the time of application.
Thomas Edison State College does not issue "Certificates of Acceptance" (Form I-20) to international students. Students who enter the U.S. on a student visa (F-1) through another college may enroll at Thomas Edison State College. However, it will be the student's responsibility to keep his/her visa status current.
College-Level Examinations: If you have taken a college-level examination, list the name of the testing agency under Colleges and Universities.
Military Documents: Submit an original DD295, AARTS or SMART transcript. Army enlisted personnel, who entered the military after Oct. 1, 1981, may submit an AARTS (Army/American Council on Education Registry Transcripts System) transcript. Air Force personnel, who entered the service after 1972, should include a transcript from the Community College of the Air Force. Members of the U.S. Coast Guard members may send transcripts from the U.S. Coast Guard Institute.
Certificates & Licenses: If you have earned a U.S. license(s) or certificate(s), it should be listed in this section. Please refer to the page on Licenses and Certificates evaluated for credit. A notarized copy of the license or certificate and a current renewal card, if appropriate, must be attached to the form.
ACE Courses Recommended for Credit: For courses completed through industry, government or special organizations and training programs, you may submit transcripts from the American Council on Education Registry of Credit Recommendations, One Dupont Circle, Washington, DC 20036-1193.
Section 5: Federal, State and Institutional Reporting Data

This section must be completely filled out. A non-refundable $75 application fee must accompany the application form.
